What are marionette lines?
Marionette lines (also known as puppet lines) are lines running from the mouth to the chin. As we age, our chin bones tend to resorb slowly which can lead to the down-turning of the corners of the mouth and the development of marionette lines. They tend to be more obvious when the face is relaxed.
Dermal fillers are used to treat marionette lines. Injection is administered into the line itself and at the corners of the mouth to soften the appearance of the line and lift the corners of the mouth.
Dermal filler results are visible immediately and last between three to twelve months depending on the filler used.
What are polynucleotides for marionette lines?
Polynucleotides are advanced biostimulators derived from DNA fragments that work beneath the skin’s surface. Unlike traditional fillers, polynucleotides don’t add volume – they kickstart your skin’s own repair mechanisms by stimulating collagen production, boosting fibroblast activity, improving elasticity and reducing inflammation.
Polynucleotides can soften the appearance of marionette lines and can be used alone or in combination with dermal fillers to reduce their appearance. If you have dermal fillers for your marionette lines, polynucleotides can act to reduce the frequency and volume of filler use and promote a lovely glow to the skin.
The results are not instant but improvement can be noted from about three weeks and continues to improve over a twelve-week period.

How do hyaluronic acid fillers work?
Dermal fillers consist of a synthetic hyaluronic acid gel. When injected into different layers of the face it can be used to reduce the appearance of sagging or loosening of the skin, folds, lines, wrinkles and to reshape areas of the face that have lost volume due to ageing.
It has the added benefit of being very hydrating, as it attracts water molecules. This allows for a plumping and firming effect and gives a more youthful and rejuvenated appearance.
Dermal filler injections are administered by using a needle or a cannula technique. Dermal fillers contain local anaesthetic within them which helps minimise discomfort during the procedure. Depending on the area treated you may also need to have topical local anaesthetic cream applied prior to treatment to minimise discomfort.
Results are visible immediately and last between three to twelve months depending on the filler used.

Frequently Asked Questions
What is the most effective treatment for marionette lines?
This really depends on the cause of your marionette lines. Every patient varies. Following a consultation, we can advise what will be most effective for you. A combination approach is often used including dermal fillers, polynucleotides and anti-wrinkle injections to the DAO muscle.
Can polynucleotides help with marionette lines?
Polynucleotides can help with marionette lines by increasing collagen and elastin production in the dermis and therefore softening the appearance of the lines.
Are polynucleotides better than fillers?
Polynucleotides are different to fillers. They are biostimulators whereas fillers are volume replacers. Polynucleotides can soften lines and create a lovely healthy glow to the skin. However, deeper or more prominent lines may require more significant volume replacement and therefore fillers are suitable for these lines.
How many sessions of polynucleotides do you need?
Usually two to three sessions are required each spaced 3 weeks apart.
How can you make marionette lines less noticeable?
This really depends on the cause of your marionette lines. Every patient varies. Following a consultation, we can advise what will be most effective for you. A combination approach is often used including dermal fillers, polynucleotides and anti-wrinkle injections to the DAO muscle.
What is the main cause of marionette lines?
Marionette lines are caused by a combination of chin recession, reduced collagen in the dermis of the skin and excess contraction of some of the lower facial muscles such as the DAO (Depressor Angularis Oris) muscle.
Why are my marionette lines getting worse?
As we age our facial bones resorb a little which leads to worsening chin recession. In addition, our collagen levels dramatically reduce and some of our lower facial muscle overact a little leading to more obvious marionette lines.
Do fillers work for marionette lines?
Yes. Hyaluronic acid dermal fillers restore lost volume beneath the corners of the mouth, lifting the skin to the same level as the lower cheek. This eliminates the shadows and creases that define marionette lines for a smoother, non-surgical facelift effect.
How long will fillers last in marionette lines?
Results typically persist for 3–12 months, depending on the type of filler, injection depth, and your metabolism.
How painful is filler in marionette lines?
Most patients report only mild discomfort—usually a brief pinch or pressure rated around 2–4 out of 10. Topical anesthetic plus cannula or needle techniques and lidocaine in the filler help keep the procedure well tolerated.
What are the side effects of marionette lines filler?
Common, temporary side effects include bruising, swelling, redness, and tenderness at the injection sites. Rare but more serious risks—such as nodules, infection, or vascular occlusion—are minimised by choosing an experienced injector and FDA-approved products.
Does filler in marionette lines look natural?
When placed by a skilled practitioner, hyaluronic acid fillers integrate seamlessly with your dermal tissue. They volumise the sunken oral commissure, smoothing lines without creating an overfilled or “pillow face” appearance.
At what age do marionette lines appear?
Marionette lines most commonly emerge around age 40 as natural collagen and elastin levels decline. However, faint creases can appear in the late 20s to 30s, especially with factors like genetics, sun exposure, and smoking.
What Our Clients Have to Say
I have had fillers in my nasolabial folds, my marionette lines and more recently, I have had fillers in my mid cheek area following weight loss on Mounjaro. The results have been absolutely amazing and really transformative.
